Role Overview As a Discharge Planner (Home Health Transitions), you will coordinate post-hospital care by arranging in-home services, equipment, and follow-up appointments. Your thorough planning and communication ensure seniors experience a seamless discharge, reducing readmissions and improving outcomes.
