Discharge Planner

About the Role

Coordinate post-hospital patient care needs from admission through discharge. Develop and maintain excellent communication with patients and their families to ensure safe discharge plans. Build strong relationships with community-based organizations and durable medical equipment (DME) companies to secure necessary post-discharge services. Lead readmission initiatives alongside the readmissions team to reduce unnecessary readmissions for the organization.

Responsibilities

• Coordinate discharge planning from admission through patient discharge.
• Communicate effectively with patients, families, and healthcare providers to develop safe discharge plans.
• Collaborate with community-based organizations and DME companies to arrange post-discharge services.
• Undergo screen training or demonstrate willingness to complete required training.
• Lead and participate in readmission reduction initiatives with the readmissions team.

Requirements

• Graduate of an accredited school of nursing OR equivalent relevant experience/education.
• Knowledgeable in current medical and allied health sciences.
• Strong verbal and written communication skills for effective collaboration with medical staff and allied health professionals.
