Discharge Planner

About the role
As a Discharge Planner, you will initiate discharge planning for patients, working closely with the treatment team to schedule appropriate aftercare appointments and communicate with referral sources. You will coordinate, conduct, and implement discharge planning for patients at the levels of care being provided upon discharge. This role also functions as part of the treatment team, maintaining effective communication with teammates, community resources, and accountability for services provided.
Responsibilities
- Provide appropriate and timely discharge planning from the facility, working closely with Utilization Review staff to maximize reimbursement and maintain appropriate census at each level of care.
- Create and maintain relationships with referral sources and community agencies providing services to persons with substance abuse and co-occurring mental health disorders, as well as identify the need to return clients to referral sources at time of discharge.
- Maintain a list of referral sources and contact information.
- Provide written discharge planning to patients and families, determining which facility or community resources should be included in the patient's discharge plan, and request the determination of level of care for placement.
- Accurately assess patient's financial resources, including insurance benefits, and coordinate transitions to providers or other referral sources as deemed appropriate.
- Document and complete discharge plans in the EHR, finalizing documentation within two working days of discharge.
- Function as a member of a multidisciplinary treatment management team and the clinical department.
- Work with patients to ensure a discharge plan, including stable housing, is set in place before discharge.
- Provide exceptional customer service to all internal/external customers with rapid response to inquiries and concerns, supporting facility internal and external customer service standards.
- Attend team meetings and complete all training courses timely as required.
- Other duties as assigned.
Requirements
- Bachelor’s Degree from an accredited college or university in a related field; applicable experience may be substituted for a degree.
- 3-5 years of clinical case management/discharge planning experience.
