Job Type: Full Time, 7:30am–4:00pm, 40 hours per week
$5,000 sign-on bonus for all clinical positions

About the Role
Seeking an energetic, team-oriented Discharge Planner for meaningful work with people admitted to our inpatient state-designated receiving facility. We provide short-term crisis stabilization and brief 1:1/group counseling in a milieu setting.
Responsibilities
- Coordinate patient discharge appointments alongside a busy inpatient psychiatric unit
- Work closely and collaboratively with patients on their plans for discharge and goals for overall treatment
- Initiate tasks and communicate effectively to a large team
- Apply knowledge of mental health and substance misuse resources in NH
- Build ongoing relationships with collateral partners
- Participate in organizational committees to enhance relationships and strengthen knowledge of available resources
Requirements
- At least one full year of experience working in the mental health field (preference given to previous experience in crisis units/environments)
- Bachelor’s degree in psychology or a related field
- Strong communication and organizational skills
- Highly reliable, focused, and experienced in customer service
