Discharge Planner

About the role

The Discharge Planner functions as a member of the interdisciplinary treatment team with primary responsibilities for discharge planning. Acts as a liaison with referral sources and other departments within the Hospital.

Responsibilities

Requirements

• Bachelor's degree in social services or related field required.
• Behavioral Health Experience is a plus.
