About the role

At RWJBarnabas Health, Discharge Coordinators play an essential role in supporting patients and families through the transition from inpatient care to the next level of care. Working collaboratively with the multidisciplinary clinical treatment team, including nursing and social work, this role helps organize discharge planning needs, coordinate referrals and placements, connect patients with community resources, and support safe, appropriate transitions. The Discharge Coordinator contributes to a compassionate, patient-centered environment while working in accordance with hospital policies, procedures, safety guidelines, and the highest achievable level of competence.

A Typical Day

  • Coordinating patient discharge planning needs in consultation with the therapist and multidisciplinary treatment team
  • Reviewing discharge planning options and evaluating alternatives with patients and their significant others to support acceptance of plans and services
  • Facilitating smooth transitions from SBBHC to another facility or a home environment
  • Arranging and coordinating pre-placement interviews and transportation for interviews or aftercare
  • Meeting with patients to discuss their needs, wants, resources, and available services
  • Providing education and connecting patients with appropriate agency and community resources
  • Consulting with the treatment team regarding projected discharge dates and the discharge plan to be pursued
  • Working closely with therapists, patients, families, and outside community mental health agencies to coordinate appropriate discharge plans
  • Documenting discharge planning involvement in the patient chart and monitoring key processes associated with discharge planning services

Requirements

  • Bachelor’s Degree in Psychology or another related Human Services field, or LPN licensure
  • 3 years of experience in an inpatient psychiatric unit

Qualifications

  • BSW preferred
  • Training and/or education in an adult psychiatric treatment setting preferred

This is a full-time, Monday through Friday position with a consistent 8:30am - 5:00pm schedule.
