Director of Social Work (Temp)

About the role

We are looking for a temporary Director of Social Work to cover our current Director's maternity leave. Position is expected to start in December. Per Diem opportunities available in the interim.

This position uniquely follows residents by supporting them through their individual continuum of care beginning at admission and throughout their transition.

Responsibilities

• Implement and document a plan of care related to the resident’s social/emotional needs in the electronic medical record.
• Collaborate with the nursing, MDS, and rehabilitation departments to facilitate discharge plans for residents.
• Inform and educate staff, residents, family, and responsible parties of the resident’s personal and property rights, as well as their rights related to health services.
• Maintain updated resources for residents and families regarding community resources, services, agencies, and organizations.
• Maintain a record of all reported concerns, assist in resolving them, and ensure appropriate follow-up and documentation.
• Be easily accessible to residents, family, and staff.
• Provide timely referrals to appropriate agencies for assistance to the resident/family (e.g., conservatorship application, Medicaid application).

Requirements

• Bachelor’s or Master’s Degree from an accredited program in Social Work or related field.
• Previous experience in a long-term care facility and familiarity with current Requirements of Participation regulations is preferred.
• Knowledgeable of admissions and discharge procedures in conjunction with all current laws, residents’ rights, and documentation requirements.
• Attention to detail, good follow-through skills, and ability to prioritize multiple tasks.
• Innovative and able to find creative ways to serve the variety of resident needs while maintaining customer focus.
• Must have a vehicle and a valid driver’s license.
• Demonstrates cooperation in assisting others and ability to work as a team.
• Patience, tact, enthusiasm, and a positive attitude toward residents and their support networks.
• Ability to remain calm in stressful situations.
• Ability to communicate with residents, staff, and family members in a professional manner.
