Director of Social Services

Annual salary $80,000 - $85,000, eligible for up to 15% bonus target. Some weekends required.

About the role

The SNF Post Acute Case Manager ensures the medically related emotional and social needs of the residents are met and maintained on an individual basis in accordance with policies and procedures and current Federal and State guidelines and regulations. This position serves as the licensed Social Worker for the Health Center.

Responsibilities

• Oversee the SNF stay from admission, focusing on patient experience, length of stay management, and transition planning to ensure high-quality care and safe, timely transition to the next level of care upon discharge.
• Coordinate and manage the transitions and care efficiencies of patients in short-term or long-term SNF care.
• Develop a comprehensive social history and assessment of the resident, including problems, strengths, special needs, preferences (social, religious, and/or cultural), and implications for the care plan.
• Assist the resident/family with the transition to the new environment, orient them to the facility, services, limitations, and resident’s rights.
• Meet regularly with patients and families at the SNF to evaluate needs, gaps in services, progress related to care plan goals, and readiness for transition to the next level of care.
• Participate in the development of the individual’s care plan, advocating for and linking patients to additional resources needed to achieve positive outcomes.
• Initiate and maintain communication and collaboration with SNF staff, physicians, and other caregiving disciplines to develop, implement, and evaluate a transition plan of care for each patient (e.g., medication reconciliation, post-discharge appointments, and services).
• Facilitate interdisciplinary meetings related to antipsychotics, gradual dose reduction efforts, trauma-informed care, and other psychosocial services as needed.
• Actively participate in departmental staff and annual policy review meetings, as well as monthly quality assurance meetings.
• Ensure any Advanced Directives are complete, maintained, and placed in resident charts.
• Complete all mandatory documentation in a timely manner as established by applicable regulations.
• Maintain confidentiality of all admission information and resident records following HIPAA and the community’s Code of Ethics.

Requirements

• Social Services Degree with 5+ years of experience or LVN with 5+ years of experience.
• Excellent oral, written, and interpersonal skills. Must present a professional manner and handle stressful situations effectively.
• Experience working with Title 22 and OBRA Regulations, including Medicare, PPS, MDS, Care Planning, and HIPAA.
• Ability to multi-task, prioritize effectively, and work closely with residents, families, and staff.
