Responsibilities
- Cook up all aspects of social services in the facility.
- Serve as a liaison between residents, families, and the care team.
- Conduct psychosocial assessments and create care plans.
- Aid residents and families with discharge planning and referrals to community resources.
- Ensure compliance with federal, state, and local regulations.
- Maintain accurate documentation and resident records.
- Provide support and guidance to social work team members (if applicable).
Requirements
- Bachelor’s Degree in Social Work or related field required; MSW preferred.
- Minimum 2 years of experience in a long-term care or healthcare setting.
- Familiarity with MDS 3.0 and care planning process.
- Strong communication, organizational, and interpersonal skills.
- Knowledge of federal and state regulations regarding social services in nursing facilities.
