About the Role
We are looking for a full-time Director of Social Services to join our clinical team and provide our guests with a care experience that will change their lives. As a friendly and compassionate individual, you will provide for the physical, psychological, and social needs of the residents, motivating them to make healthy adjustments to the skilled nursing center. In this role, you will assume administrative authority, responsibility, and accountability to provide medically-related social services and implement interventions that achieve treatment goals, address resident needs, and enhance quality of life.
Responsibilities
- Coordinate admissions and discharge procedures.
- Attend Interdisciplinary Team and Unit Care conferences.
- Assess residents for psychosocial groups and supervise implementation and documentation.
- Interview new residents and assess their psycho-social condition.
- Participate in Interdisciplinary Team Meetings and Care Conferences, providing information on individual resident’s psycho-social condition and prospects for discharge or movement within the facility.
- Chart relevant social service information into resident’s records.
- Oversee and co-manage the psychoactive committee review with the Director of Nursing.
- Discuss problems encountered by residents and assist with implementing solutions.
- Conduct follow-up procedures to ensure resident satisfaction.
- Address roommate problems and work toward resolution.
- Make recommendations regarding a resident’s ability to function effectively at home or at other levels within the facility.
- Assist with discharge planning for homeward-bound residents.
- Attend in-service programs offered by the Nursing Home and deliver in-services pertaining to Social Service functions as requested.
- Make all counseling decisions and referrals to outside agencies on a regular basis.
- Maintain contact with staff from all departments to address diverse resident problems.
- Maintain ongoing communication with the Director and Assistant Director of Nursing to exchange information on resident status.
Requirements
- Bachelor’s degree in Social Work, Human Services, Sociology, Education, Psychology, Rehabilitation/Counseling, or a related field (required). Master’s degree (a plus).
- Minimum one year of experience working in skilled nursing (required).
- Management experience.
- SSD Certification.
