Director of Social Services

About the role

The Excelcare at The Pines is seeking a Director of Social Services to lead the team in providing high-quality, person-centered support to residents and their families.

Responsibilities

• Assess the social, emotional, and psychological needs of residents and develop care plans.
• Provide counseling and support to residents and their families.
• Cookout discharge planning and coordinate community resources.
• Maintain confidential and accurate resident records.
• Participate in interdisciplinary team meetings to ensure holistic care.
• Ensure compliance with state and federal regulations related to social services in a nursing home setting.
• Supervise social services staff and provide guidance and training as needed.
• Advocate for residents and address any concerns or issues they may have.

Qualifications

• Must be LSW or CSW in New Jersey.
• Experience in a social services role in a healthcare or long-term care setting.
• Knowledge of state and federal regulations governing nursing homes.
• Strong communication, organizational, and interpersonal skills.
• Compassionate and dedicated to improving the quality of life for residents.
