Director of Social Services

Location: Weymouth, MA – CareOne at Weymouth

Full-time, all shifts • $85,000 – $110,000 per year (commensurate with experience, qualifications, credentials, employment status, and work schedule)

Requirements

• Master’s Degree in Social Work
• Current and active LSW (Licensed Social Worker) licensure in the state of practice
• Experience in one or more practice settings: Sub-Acute Care, Short-Term Rehab, Assisted Living, Skilled Nursing, LTACH, Long-Term Care
• Adherence to CareOne Social Service Policies and Procedures Manual, State and Federal guidelines, and the National Association of Social Workers Code of Ethics

Responsibilities

• Supervise social service staff and oversee direct services, advocacy, counseling, service coordination, education, in-servicing, policy and program development, and community relations
• Act as a resource to other Social Workers
• Assist with recruiting, interviewing, and hiring new personnel; orient new staff
• Establish standards of practice, update and revise Policy and Provision manual, implement peer review process, and validate competencies
• Facilitate regular meetings and communication channels among Social Workers
• Participate in Center-based Quality Improvement initiatives and submit monthly reports to Regional Director of Operations
• Establish partnerships with the Office of the Ombudsman
• Plan and direct a comprehensive social service program supporting residents and families
• Participate in an interdisciplinary team approach to resident care
• Provide therapeutic intervention to help residents cope with social and psychological aspects of illness, impairment, disability, institutionalization, and loss
• Maintain written documentation in resident medical records per CareOne, State, and Federal guidelines, including initial assessments, quarterly updates, and ongoing documentation for short-term residents
• Develop the social service component of each resident’s plan of care, identifying problems, goals, and approaches; coordinate interdisciplinary care planning conferences
• Ensure residents and families have input into the care planning process
• Act as liaison between residents, families, outside agencies, and the Center Administrator to uphold resident rights
• Conduct family meetings as needed
• Perform administrative duties, including completing necessary forms and reports under the direction of the Administrator
• Facilitate discharge planning, including development of organized discharge plans for all residents
• Promote resident safety, minimize risks, and ensure compliance with legal, safety, health, fire, and sanitation codes
