Director of Social Services
Location: Randolph, MA | Facility: CareOne at Randolph | Full Time, all shifts
Pay
Minimum $85,000 – Maximum $110,000. Compensation will be based on experience, qualifications, credentials, employment status, and work schedule.
Requirements
- Master’s degree in social work
- Current and active LSW (Licensed Social Worker) licensure as required by the State in which you are working
- Experience in practice settings such as sub-acute care, short-term rehab, assisted living, skilled nursing, LTACH, or long-term care
Responsibilities
- Abide by and practice according to CareOne Social Service Policies and Procedures Manual, State and Federal guidelines, and the National Association of Social Workers Code of Ethics
- Supervise social service staff and oversee direct services, advocacy, counseling, service coordination, education, in-servicing, policy and program development, and community relations
- Act as a resource to other social workers
- Assist with recruiting, interviewing, and hiring of new personnel
- Orient new staff and establish standards of practice for role uniformity
- Update and revise Policy and Provision manual
- Implement a peer review process to validate competencies
- Establish communication channels with other social workers through regular meetings
- Participate in center-based Quality Improvements
- Submit monthly reports summarizing issues
- Establish partnerships with the Office of the Ombudsman
- Plan and direct a comprehensive social service program providing support services for residents and families
- Participate in an interdisciplinary team approach to serve the needs of the center
- Provide therapeutic intervention to help residents cope with social and psychological aspects of their illnesses, impairments, or disabilities
- Maintain written documentation in residents’ medical records per CareOne policy, including initial evaluations and quarterly assessments
- Develop the social service component of residents’ care plans, ensuring resident and family input
- Coordinate interdisciplinary care planning conferences
- Act as liaison between residents, families, outside agencies, and the Center Administrator to maintain residents’ rights
- Conduct family meetings as needed
- Perform administrative tasks such as completing necessary forms and reports
- Facilitate discharge planning, including developing organized discharge plans
- Ensure resident safety and center compliance with legal, safety, health, fire, and sanitation codes
