About the role
Aviata at Lakeside Oaks is searching for a Director of Social Services to join our resident- and family-focused community. The primary purpose of this position is to provide services that respond to the emotional needs of residents and their families, assist with resident admissions, and support the referral process.
Responsibilities
- Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Executive Director/LNHA.
- Implement policies and procedures for the identification of medically related social and emotional needs of residents.
- Participate in community planning related to the interests of the facility and the services and needs of residents and families.
- Participate in discharge planning; development and implementation of social care plans; and resident assessments.
- Provide consultation to staff, community agencies, and others to address resident needs through social service programs.
- Review and develop a plan of correction for social service deficiencies noted during survey inspections and provide a written copy to the Executive Director/LNHA.
- Maintain a reference library of written material, laws, standards of practice, and other resources to ensure compliance with current standards and regulations.
- Conduct and document social services evaluations, including identification of resident problems and needs.
- Maintain communications with caseworkers.
- Provide social work consultation to residents, families, and staff as required.
- Drive innovation in healthcare by creating and implementing new ideas and concepts to improve systems and processes.
Requirements
- Bachelor’s Degree and either a LSW or MSW from an approved school of Social Work.
- Minimum of one (1) year of experience.
- Demonstrated ability to assess resident needs, develop and implement care plans, and coordinate discharge planning.
- Knowledge of accreditation standards and compliance requirements.
- Membership in good standing with the National Association of Social Workers.
