Continuum Rehab Group is seeking a Director of Rehab/COTA to join our in‑house therapy team serving Skilled Nursing/Long Term Care facilities in Whitesboro, TX. Responsibilities

• Manage and direct rehabilitation services within the skilled nursing facility, including Physical Therapy, Occupational Therapy, and Speech Therapy.
• Oversee, direct, and coordinate day‑to‑day operational activities and programs to deliver cost‑effective, high‑quality services.
• Ensure efficient services that meet the needs of patients, physicians, clinical partners, and staff.
• Assume ultimate responsibility for rehabilitation service operations within the assigned facility.
• Maintain service and operational excellence to achieve quality outcomes.
• Attend required meetings such as Morning Meeting, Utilization Review (UR) meeting, Triple Check, QAPI, PDPM, and Resident at Risk.
