Job Summary

The Director of Health Services (DHS) oversees the health and well-being of residents in an Assisted Living Facility, Memory Care Facility, or Resident Care Facility. Key responsibilities include conducting resident assessments, developing and implementing care plans, ensuring regulatory compliance, managing medication, leading staff, and fostering collaboration.

Responsibilities

  • Conduct thorough assessments of new and existing residents to identify healthcare needs, preferences, and goals.
  • Develop, implement, and evaluate individualized, person-centered care plans in collaboration with residents, families, and interdisciplinary team members.
  • Maintain accurate and up-to-date nursing documentation in compliance with state regulations and facility policies.
  • Ensure the facility adheres to all state and federal regulations, healthcare standards, and best practices.
  • Oversee the Health Service Department, completing audits of medication administration for compliance with state and federal regulations.
  • Provide direction, mentorship, and support to medication and care staff.
  • Assist with training on nursing protocols, medication administration, and emergency procedures.
  • Foster open communication with residents, families, staff, and outside providers to ensure coordinated, collaborative care.
  • Coordinate with outside providers, including physicians, therapists, and hospitals, to ensure continuity of care and address any special healthcare needs.
  • Handle dynamic and evolving healthcare situations by applying critical thinking skills and effectively responding to emergencies both in person and remotely.
  • Participate in continuous quality improvement initiatives aimed at enhancing care delivery, resident satisfaction, and staff engagement.
  • Monitor key performance indicators and identify areas for improvement.
  • Participate in on-call rotation for after-hours, weekends, and emergencies.

Requirements and Qualifications

  • Must possess a nursing degree from an accredited college or university.
  • Must possess a current, unencumbered, active license to practice as a Registered Nurse / RN in this state.
  • BSN preferred.
  • Nursing experience in assisted living, Alzheimer/dementia, long term care, geriatric rehab, sub-acute and or critical care clinical setting.
  • Thorough knowledge of CMS and State Regulations regarding patient care in an ALF/IL/MC facility.
  • Experience in electronic medical records documentation.
  • Strong leadership and communication skills.
  • Ability to design and deliver effective training programs.
  • Knowledge of State and Federal regulations and compliance requirements.
  • Excellent organizational and time management skills.
  • Maintain and have an active CPR/First Aid Certificate during employment.
  • Must be able to read, write, and speak English fluently.
