Major Duties And Responsibilities

  • Analyzes and critically thinks about the organization's care model to guide patient care.
  • Maintains the standard of clinical care, using diagnostic testing appropriately and cost-effectively, and providing appropriate treatment.
  • Assesses barriers in patients' social environments that may affect health outcomes and works with the business intelligence team to promote data-driven decisions.
  • Create and implement new quality initiatives and related PDSAs or fishbone diagrams, as needed.

Professionalism

  • Demonstrates professionalism and integrity in all aspects of patient care and collaboration.
  • Fosters a collaborative work environment.

Communication

  • Advocates for referral to ASTRx for HIV patients.
  • Responds promptly to queries from sponsors.
  • Participates in internal committees and work groups as requested.
  • Leads the monthly ID Committee meeting.
  • Represents Chase Brexton at community, quality, and TA meetings including but not limited to Planning Council, Baltimore EMA Quality Improvement Collaborative, and Maryland HIV Planning Group.
  • Participates in monthly and quarterly conference calls with Program Officers as the Ryan White Quality Lead for the organization.

Patient Focus

  • Effectively engages patients in their care.
  • Fosters an environment of excellent customer service.
  • Works closely with SWOD and the patient advisory council (PAC).
  • Ensures quality and compliance with regulatory standards.
  • Provides clinical training, leadership, and mentorship to staff providers, residents, and externs.
  • Promotes excellence in patient care and outcomes through collaborative work with other departments.
  • Leads efforts to develop and establish programs to improve ID quality for the communities served.
  • Collaborates with teams to assess and enhance program effectiveness, patient satisfaction, and access to care.

Compliance, Policy, and Procedure

  • Shows meaningful progress toward quality improvement goals.
  • Keeps current with organizational policies and procedures and puts them into action accordingly.
  • Adheres to RW goals and requirements and implements PDSAs.
  • Ensures adherence to administrative policies and procedures.

Workplace Computers and Equipment

  • Maintains complete, legible, and comprehensive documentation.
  • Utilizes EMR resources to manage patient care, flags, protocols, and referrals.

Planning and Organizing

  • Timely with respect to completion of necessary paperwork such as disability forms.
  • Reads and responds to e-mail in a timely manner.
  • Covers grant deliverables and performance metrics with Ryan White Program Coordinator including but not limited to work plans, progress reports, CQM plan, CQM narrative, etc.

Strategic Leadership

  • Provides direction to the organization with respect to ID related matters.
  • Fulfills the role of CBHC's clinical consultant in ID.
  • Keeps CBHC clinical staff aware of updates to ID guidelines and new research findings through regular presentations at staff meetings.
  • Ensures that medical staff and other interested/involved departments (nursing, SWOD) are aware of current clinical trials and changes in guidelines.
  • Represents CBHC to the medical and patient communities, as well as the community at-large, as an expert in HIV Medicine.

Teamwork

  • Collaborates with all other departments across the organization to uphold quality standards and address patient satisfaction in a timely manner.
  • Works closely with leadership, finance, information technology, quality assurance, infection control, and other departments to ensure:
  • Accurate and compliant health records.
  • Timely staff review and audits.
  • Staff compliance with protocols and documentation standards.

Performance Management

  • Completes evaluations of staff in a timely manner.
  • Supervises and supports all clinical providers reporting to the DoID.
  • Sets and communicates clear performance expectations, goals, and objectives for team members.
  • Identifies training needs and supports ongoing development opportunities for staff.
  • Fosters a culture of accountability, collaboration, and continuous quality improvement.
  • Ensures staff adhere to clinical standards, safety protocols, and documentation requirements.
