Responsibilities
- Provide ongoing support during overnight and morning hours (self‑care routines: toileting, feeding, showering, dressing, diapering); address health, safety, alternate positioning and overall well‑being to maintain a high quality of life.
- Ensure a safe and clean home by assisting with household chores (e.g., laundry) and participating as needed.
- Follow all treatment plans and utilize adaptive and safety equipment to promote independence, health, safety and well‑being.
- Report and document any events that may hinder health, safety or well‑being (allegations of abuse, injuries, broken equipment, falls, etc.).
- Act as a mandated reporter to the Justice Center when there is reasonable cause to suspect abuse or neglect; follow agency incident policies.
- Adhere to Federal and New York State regulations, agency policies, employee handbook and training requirements.
- Successfully complete and maintain AMAP certification; assist with medication administration per AMAP policy.
- Arrive at the start of the scheduled shift; follow agency call‑off and overtime procedures.
- Support morning meal preparation, teaching diet adherence, portion control and cooking skills.
- Wake and greet individuals with a soft, kind tone; support their preferred morning routine.
- Accompany or transport individuals using safe driving practices to day programs or job locations.
- Implement and document treatment plans and agency materials via Medisked or paper (overnight checklists, goals, valued outcomes, medical case notes, communication logs, BM/menses charts, sleep charts, seizure logs, behavioral data, etc.).
- Participate as an interdisciplinary team member in meetings to discuss progress, goal achievement and share information.
- Mentor and role‑model new Direct Support Staff by demonstrating supportive routines and reviewing training manuals.
- Perform other duties as assigned by supervisor.
Requirements
- High School Diploma or GED preferred.
- Qualified New York State Driver’s license meeting agency insurance standards.
- Successful completion and maintenance of AMAP status.
- Ability to communicate and understand written and oral directives/information.
- Excellent team player with a positive attitude.
- Must pass fingerprint/criminal background check (position involves regular unsupervised physical contact).
