About the role
Practices and promotes the agency’s mission statement in every interaction. Provides ongoing support to people during overnight and morning hours, including self-care routines (toileting, feeding, showering, dressing, diapering), health, safety, alternate positioning, and overall well-being to ensure a high quality of life is maintained.
Responsibilities
- Provide ongoing support during overnight and morning hours (e.g., self-care routines, health, safety, positioning, and well-being).
- Ensure a safe and clean home by assisting with household chores (e.g., laundry) and participating as needed.
- Follow all treatment plans and use adaptive and safety equipment to promote independence and ensure health, safety, and well-being.
- Report and document events that may hinder health, safety, or well-being (e.g., allegations of abuse, injuries, broken equipment, falls).
- Report to the Justice Center as a mandated reporter if abuse or neglect is suspected; follow all agency incident policies and procedures.
- Adhere to Federal and New York State regulations, agency policies, the employee handbook, and training requirements.
- Complete and maintain AMAP certification; assist people with medication administration per AMAP policy and procedures.
- Arrive to work on time; adhere to call-off and overtime policies to ensure quality support.
- Support morning routines, including meal preparation (e.g., diet adherence, portion control, cooking skills).
- Wake and greet people with a soft and kind tone; support preferred morning routines.
- Assist with medication administration per AMAP policy and procedures.
- Transport people to day programs or job locations using safe driving practices.
- Implement and document treatment plans and agency materials (e.g., overnight checklists, goals, medical case notes, communication logs, charts) via Medisked or paper documentation.
- Serve on the inter-disciplinary team by attending meetings to discuss progress, goals, and share information.
- Mentor and model supportive routines for new Direct Support Staff; review training manuals to ensure readiness.
Requirements
- High School Diploma or GED preferred.
- Qualified New York State Driver’s license meeting agency insurance standards.
- Successful completion and maintenance of AMAP status.
- Ability to communicate and understand written and oral directives; excellent team player with a positive attitude.
- Qualified driver status required for overnight shifts (11 pm–9 am) to assist with morning transportation.
