Responsibilities
- Build positive, respectful relationships with the individuals served.
- Help participants work toward the goals in their person-centered support plans.
- Lead and take part in recreational, educational, vocational, and community-based activities.
- Encourage independence while providing individualized support as needed.
- Aid in personal care while preserving each person's dignity and privacy.
- Support participants during community outings, volunteer activities, and recreational events.
- Promote communication, social skills, decision-making, and self-advocacy.
- Maintain a safe, clean, welcoming environment.
- Complete daily documentation and electronic service notes accurately.
- Communicate effectively with supervisors, families, guardians, case managers, and other team members.
- Respond appropriately during emergencies using approved procedures.
- Follow all Medicaid Waiver, CARF, HIPAA, and DDSi policies.
Requirements
- A high school diploma or GED.
- At least 18 years of age.
- A valid driver's license with an acceptable driving record.
- Ability to pass required background screenings.
- Ability to obtain CPR/First Aid certification (training provided).
Preferred Qualifications
- Previous experience working with individuals with disabilities (preferred, not required — training is provided).
Physical Requirements
- Standing and walking throughout the day.
- Assisting individuals with mobility when necessary.
- Lifting up to 50 pounds.
- Bending, reaching, and transferring individuals using safe techniques.
- Driving participants to community activities.
