Summary
Position responsible for direct, basic and supportive care to consumers with intellectual and developmental disabilities within an assigned program.
Essential Duties and Responsibilities
- Provides training, health monitoring, assistance with activities of daily living and transportation for adults with intellectual and developmental disabilities.
- Provides oversight and intervention to consumers that are experiencing a major behavioral health crisis.
- Serves as a member of the residential/day support services team and attends all designated meetings.
- Maintains daily logs and records of patient care, condition, progress or problems to report and discuss observations with supervisor or Service Coordinator.
- Maintains consumer safety needs, including physical, medical, behavioral and psychiatric.
- Maintains consumer safety by observing overall health and wellness.
- Documents consumer functional and dysfunctional behavior according to individual program plan.
- Maintains facility to ensure sanitary conditions.
- Affords assistance with cooking, laundry and other general household activities necessary to maintain sanitary conditions and general housekeeping upkeep.
- Accompanies consumer to doctor’s offices and on other trips outside the home, providing transportation, assistance and companionship.
- Complies with federal, state and company policies, procedures and regulations.
- Maintains confidentiality of all information and complies with HIPAA regulations.
- Demonstrates knowledge of and supports Westbrook Health Services mission, vision, values, policies and procedures, operating instructions, confidentiality standards and code of ethical behavior.
Requirements
- Works well in a team environment
- Work with others in a professional, cooperative, ethical, respectful, and effective manner.
- Has a valid driver’s license and current auto insurance.
- Must pass Criminal Investigation Background and DHHR background checks as well as post offer drug screen.
- Successful completion of Title XIX waiver and other Westbrook Health Services training as applicable (provided).
Qualification Requirements
- Physical Requirements: The employee is frequently required to walk; use hands to finger, handle, or feel; and reach forward with hands and arms. The employee is occasionally required to sit and stoop, kneel, or crouch. The employee must occasionally lift and/or move up to 50 pounds.
