About the role

This position provides direct support and training to individuals with disabilities in community-based, employment settings, including OPTS Centers, supported employment sites, and individual job placements. It involves working closely with Case Managers, Program Directors, and Job Placement staff to promote individual readiness for community-based employment.

Responsibilities

  • Provide support to individuals in a 1:1 or group format as defined in the ISP.
  • Work in cooperation with Case Managers to develop member skills in self-care, self-preservation, expressive communication, self-direction, and activities of daily living.
  • Collaborate with coworkers, individuals, staff from other agencies, outside professionals, and consultants to promote growth and independence for individuals in our programs.
  • Instruct, supervise, and evaluate individuals in all areas of community living as outlined in each support plan and record data on all instructional programs.
  • Maintain the program in a clean, orderly condition as outlined by Supervisor, including tasks like vacuuming, dusting, cleaning floors, bathrooms, tables, counters, windows, and laundry.
  • Assist and support individuals as needed with personal care responsibilities, including diet, exercise, meal preparation, bathing, shaving, dressing, toileting, and physical direct care tasks.
  • Implement all plans and treatments as outlined, including Occupational Therapy (OT), Physical Therapy (PT), Behavioral Nursing, and Speech therapy (may include physical intervention).
  • Adhere to all established lines of communication, maintain a positive and proactive attitude, and represent a positive image with coworkers, families, and referral agencies.
  • Write reports, utilize professional documentation skills, attend required meetings, adhere to program routines, and follow customer/host company schedules and needs.
  • Dress in a manner that reflects the corporate culture of the specific business in which individuals are placed and adhere to agency safety standards.
  • Perform additional duties or substitute for others as required, adhere to RTR policies and procedures, maintain current training certificates, and perform additional duties as assigned.
  • May include clearing snow and ice from walkways, vehicles, parking lots, and roads using equipment like shovels and snow blowers while applying deicing agents to ensure safety.

Qualifications

  • High School Diploma or equivalent.
  • Valid Driver’s License.
  • Basic knowledge of Microsoft Word and the internet.
  • Successful completion of all state and agency mandated trainings.
  • Willingness to authorize a Criminal History Background Check (CORI) and release of information concerning allegations from Disabled Persons Protection Commission (DPPC), Department of Developmental Services (DDS), and other agencies.
  • Ability to perform each essential function satisfactorily.
  • Reasonable accommodations may be made to enable individuals with disabilities to perform essential functions.
  • Fluency in English and ability to read, write, and communicate effectively.
