About the role

Part-time Dietitian position, 24 hours per week with flexible schedules on Monday and Friday.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and develop a plan of care within the electronic medical record.
• Develop, implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake, and comorbidities.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to develop a nutritional health plan for the patient's overall wellbeing.
• Adhere to facility policy and procedures and to CMS, State, and Federal regulations.
• Assist and participate in interdisciplinary care conferences.
• Assist the dietitian in duties as assigned.

Requirements

• Registered by the Commission on Dietetic Registration.
• Hold a certification/license in good standing within the state of practice.
• Knowledge of the state survey process and compliance with nutrition care regulations.
• Strong organization and communication skills.

Pay

$30.00–$35.00 per hour, with competitive wages based on experience.
