About the role

The role involves incorporating individual and interpersonal level nutritional educational interventions that create changes in knowledge, attitudes, behavior, and health outcomes among individuals, families, or small, targeted groups within a community setting.

Responsibilities

  • Manage El Poder Nutrition and Diabetes Programs and Initiatives
  • Teach and implement diabetes prevention and management program for the Hispanic community in a group setting
  • Work closely with program coordinator to develop the annual curriculum for the 12 week and 10 week El Poder program
  • Provide nutritional assessment for all El Poder participants during initial and final program sessions
  • Develop educational materials and PowerPoint presentations in Spanish
  • Aid in the preparation of nutrition lesson plans that include portion control, MyPlate method, etc
  • Aid program coordinator with data collection including screening results, pre and post test
  • Document and screen for social determinants of health seen among program participants in order to raise awareness of systemic barriers to community stakeholders, and funders
  • Periodically reassesses participants needs, effectiveness of interventions, and attainment of chosen outcomes
  • Provide information, education and direction to program coordinator to support program participant's needs
  • Measurements methods: schedules and calendar of events and attendance sheets annually, review all program evaluations and make changes to curriculum and programs according to data collected
  • Nutrition Counseling: provide both individual and group nutrition counseling for El Poder participants, assess nutritional needs and develop nutritional care plans for individuals
  • Maintain referring physicians informed of patients nutritional care plan in writing in a timely manner
  • Supervisory observation: develop culturally appropriate curriculum that is evidence-based, and use linguistically appropriate materials, based on the recommendations of the Academy of Nutrition and Dietetics
  • Promote programs in the community by utilizing flyers, social media, etc
  • Support with programming the nutritional objectives outlined in the Community Health Improvement Plan (CHIP)
  • Complete required documents and reports, if needed to be presented yearly grant renewal
  • Assist with final grant report based on set goals, objectives and outcomes
  • Report program activities in CBISA, when necessary
  • Event registration and program representation: represents and promotes Community Health at internal and external events i.e. (health screenings, exhibits, community functions)
  • Supports community events during non-regular working hours i.e. (after 5 pm; and on weekends)
  • Collaborates with colleagues on department initiatives and projects
  • Professional Development: demonstrates professional growth and development on the job as evidenced by completing the following: attends all mandatory hospital/department in-service meetings, attends conferences and in-service to maintain competency and basic skills in assessment, counseling and planning nutrition care for pediatric, adolescent, adult and geriatric patients, maintains all documents and necessary records of in services and continuing education lectures, observes the limits of confidentiality and privacy issues, adhering to best practice standards, keeps abreast of all pertinent federal, state, and RWJUH regulations, laws, and policies as they presently exist and as they change or are modified

Qualifications

  • Bachelor’s or Master’s Degree in Nutrition and Health Sciences, Dietetics or equivalent
  • Academy of Nutrition and Dietetics licensure required
  • Licensure applicants must complete a one year internship at minimum
  • Bilingual in English and Spanish required
  • Previous community education experience strongly preferred
  • Certified Registered Dietitian or RDN eligible
