Dialysis Registered Nurse, Home Hemo & Peritoneal RN

We seek a Home Therapy Registered Nurse to provide patient training and ongoing support for those choosing a home dialysis modality. Responsibilities

• Comply with federal, state, and local laws and regulations, including health care professionals' practice act requirements for home dialysis programs and patient care.
• Train patients (and/or care partners) in self-care Peritoneal Dialysis or Hemodialysis, ensuring compliance with federal and state regulations for Registered Nurse qualifications.
• Conduct initial and annual home visits to assess the patient's home environment; perform more frequent assessments as needed.
• Routinely evaluate patient performance and home environment to improve care.
• Communicate patient assessment results, reassessments, and ongoing monitoring to physicians, team members, and others as appropriate.
• Document all nursing services in the Electronic Medical Record, including training sessions, in-person interactions, and phone conversations, ensuring continuity of care.
• Participate in infection control monitoring, implementation, and recording as requested.
• Familiarize yourself with emergency equipment and procedures; regularly review Emergency Preparedness with home patients.
• Ensure clinic and patient supply inventory aligns with patient prescriptions; place orders in a timely manner to avoid fees and penalties.
• Track and trend program and patient performance to improve outcomes using critical thinking and root cause analysis; assist in obtaining data for continuous quality improvement.
• Obtain and communicate routine and non-routine laboratory tests, including critical lab values, to the responsible physician or extender.
• Complete and document monthly reviews of patient medication profiles; administer medications as ordered by the physician.
• Perform on-call nursing services, including nights and weekends, on a rotational basis as needed or assigned.
• Be flexible with staffing locations and hours to accommodate patient and program needs.
• Maintain regular and reliable attendance.
• Home Hemodialysis Only:
  • Coordinate home evaluations with the technical services department to ensure electrical, plumbing, and drainage requirements are met before patient acceptance.
  • Ensure required water sampling is completed per policy; monitor and report water sample and culture results.

Requirements

• Current RN license in the applicable state; must be maintained as current and in good standing.
• 12 months of clinical nursing experience and 3 months of experience in the specific home dialysis modality (6 months in Texas).
• Previous experience in providing nursing care to patients on maintenance dialysis preferred.
• CPR certification required prior to patient care assignment.
• Ability to distinguish all primary colors.
• Successful completion of the Home Therapy Nurse Orientation program; maintain annual demonstration of skills and competency for the assigned modality.
• Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
• Basic computer skills; proficiency in all USRC clinical applications required within 90 days of hire.
• Meet any additional practice requirements for the applicable state.
• Ability to meet minimum hiring standards, including background clearances and orientation requirements if applicable.
