Diabetes Educator, Endocrinology

About the role

Wentworth Health Partners (WHP), an affiliate of Wentworth-Douglass Hospital and member of Mass General Brigham Medical Group, is expanding its Diabetes Services team. They are currently hiring Diabetes Educators for both 30-hour and 32-hour part-time opportunities.

Responsibilities

• Conduct assessments to determine the educational needs and readiness of patients to learn about diabetes management.
• Develop and implement individualized education plans addressing diabetes self-management, including medication management, nutrition, physical activity, and blood glucose monitoring.
• Facilitate group education classes on diabetes management, prevention, and lifestyle modifications.
• Create educational materials and resources to enhance patient understanding and engagement.
• Provide ongoing support and counseling to patients and their families, addressing the emotional and psychological aspects of living with diabetes.
• Encourage patient adherence to treatment plans and empower them to actively participate in their health.

Qualifications

• Education: Associate's Degree in a related field required; Bachelor's Degree preferred; Master's Degree preferred.
• Licenses and Credentials: Certified Diabetes Educator [CDE/CDCES] certification required.
• Experience: 2-3 years of experience in health and nutrition related to diabetes education, counseling, or related roles.
