About The Role As a key member of our Primary Health Team, you will: Deliver high‑quality diabetes education to individuals, families, and groupsProvide outreach services to rural and remote communitiesBuild strong partnerships with GPs, Aboriginal Health Services and other stakeholdersSupport clients through coordinated, multidisciplinary careMentor students and contribute to best-practice modelsUse your expertise to improve accessibility and health outcomes for priority populations This role offers variety, autonomy, and the opportunity to make a genuine difference. What You’ll Bring Tertiary qualification in Nursing, Dietetics, Podiatry, Pharmacy, or Exercise PhysiologyAHPRA registration (or eligibility)Graduate Certificate in Diabetes Education (or willingness to obtain)ADEA credentialling (or working towards it)Strong communication skills with the ability to engage across culturesAbility to work both independently and within a multidisciplinary teamExcellent organisational and digital skillsCurrent Driver’s Licence Desirable Experience in rural health, Aboriginal and Torres Strait Islander communities, telehealth, health promotion or research.
