About the Role
The Diabetes Educator provides personalized education to clients with diabetes across all ages, assessing their needs and developing tailored education plans that address psychosocial, economic, and language barriers. This role involves conducting health and education assessments, implementing and evaluating education strategies in both classroom and individual outpatient settings, and collaborating with RNs and RDs. Ongoing professional development and community outreach participation are required.
Responsibilities
- Assess client needs and develop tailored diabetes education plans addressing psychosocial, economic, and language barriers.
- Conduct health and education assessments for clients with diabetes.
- Implement and evaluate education strategies in classroom and individual outpatient settings.
- Collaborate with registered nurses (RNs) and registered dietitians (RDs).
- Participate in ongoing professional development and community outreach.
Requirements
- Oregon Dietitian License and CDR Registered Dietitian certification at hire.
- Minimum three years of recent experience in diabetes treatment and education.
- Knowledge of adult learning theories and clinical diabetes management.
Preferred Qualifications
- Credentials in adult learning principles.
- Certification in Diabetes Education (CDE).
- Experience working with diverse populations.
