Essential Functions
Patient and Caregiver Support
- Work with Palliative and Hospice admission teams to identify appropriate patients for dementia care.
- Provides individualized dementia education and coaching to patients, families, and caregivers across disease stages, from early palliative involvement through end-of-life hospice care.
- Coaches caregivers in practical strategies for managing dementia-related symptoms, including agitation, anxiety, sleep disturbance, communication challenges, and behavioral changes.
- Supports families with anticipatory guidance related to disease progression, functional decline, and end-of-life changes.
- Consults with Palliative and Hospice Interdisciplinary Team (IDT) to trouble-shoot and provide guidance and direction for care.
- Participates and assist with leadership of the Dementia Resource Team as a key resource within the organization.
Community Education and Awareness
- The Dementia Coach is the point person for community education and outreach regarding the dementia support program and related programs.
- Provides educational programs
- Facilitates community access to related programs (VDT, Palliative, Hospice, etc.)
- Utilizes provided Teepa Snow Positive Approach to Care training to provide educational opportunities for community and within Choices Healthcare organization.
Virtual Dementia Tour Leadership (VDT)
- Utilizes provided Second Wind Dreams (Virtual Dementia Tour) training to lead a team of staff to operationalize and provide VDT training within the organization (staff and volunteers).
- Leads efforts to grow the VDT program to be able to share VDT with the community
Qualifications
- Associates degree or equivalent in related field required
- Teepa Snow Dementia Coach certification required within 1 year from date of hire
- Virtual Dementia Tour Certified trainer required within 1 year from date of hire
- 1 year of experience working with patients and caregivers living with dementia required
- Must be able to speak and lead in a group education setting (large and small audience)
Schedule
Full Time, 40 hrs. Monday - Friday 8am - 5pm. Must be able to flex your hours on occasion. Must be available to travel to other locations (anywhere in our service territory).
