Decentralized Internal Medicine Pharmacist
Job Summary
The Internal Medicine Pharmacist at Memorial Health University Medical Center provides pharmaceutical care including computer order entry, drug distribution, evaluation of medication orders for appropriateness, drug interactions, and allergies, and therapeutic monitoring of drugs.
Responsibilities
- Cookbook and order verification of medication orders
- Resolution of medication related problems
- Preparation of parenteral products
- Coordination of drug distribution activities
- Supervision of pharmacy technicians
- Performing pharmacy clinical activities such as pharmacokinetic consults, monitoring targeted drugs, antimicrobial therapy recommendations, and evaluating appropriateness of drug therapy
- Serving as a source of drug information to medical and nursing staffs
- Responding to Code Blue Alerts
- Performing patient care unit inspections as required to meet regulatory requirements
- Minimizing errors in the medication use process using S.T.A.R. and other error prevention techniques
- Following special handling procedures related to high-risk, high-alert medications and patient populations
Qualifications
- Clear and active Georgia pharmacist license
- Graduate of ACPE-accredited School of Pharmacy; PharmD degree preferred
- PGY-1 residency and/or Board Certification
