Day Shift LPN
Responsibilities
- Provide direct patient care, including administering medications, treating wounds, and performing vital signs
- Develop and implement individualized care plans in collaboration with the healthcare team
- Monitor and document patient progress, identifying areas for improvement and communicating changes to the interdisciplinary team
- Participate in shift report, handovers, and team huddles to ensure seamless care transitions
- Maintain accurate and thorough records of patient care, including medication administration and treatment plans
- Demonstrate professionalism, empathy, and understanding in all interactions with patients, families, and colleagues

Requirements
- Must be a Licensed Practical Nurse (LPN) in the state of Oklahoma
