Position Summary

Chrysalis provides support services for at-risk adults and/or juveniles with developmental and intellectual disabilities. This position involves working in stressful and safety-sensitive environments, making critical decisions about care and treatment, and communicating these decisions effectively.

Essential Duties and Responsibilities

  • Punctuality and consistent attendance are required.
  • Stay alert, responsive, and communicate effectively with individuals.
  • Teach daily living skills and plan activities.
  • Create opportunities for individuals to improve their quality of life.
  • Maintain a clean, healthy, and safe environment.
  • Implement and follow Person-Centered Support Plans and Behavioral Plans.
  • Supervise individuals according to the Supervision Guidelines and support their choices.
  • Assist and supervise individuals with taking appropriate medication.
  • Complete necessary administrative tasks within established timeframes.
  • Complete all required documentation for Chrysalis and the state.
  • Maintain communication with the Manager regarding individual needs.
  • Follow the Provider Code of Conduct and state standards for abuse, neglect, and exploitation.
  • Aid the Director and Day Program Manager in implementing behavior plans and Supervision Guidelines.
  • Help with employee schedules.
  • Monitor paperwork related to individuals in groups (including attendance and data collection).
  • Use paperwork to fill out and submit MPRs for individual groups.
  • Attend monthly meetings with the Day Program Manager to discuss topics.
  • Perform other duties as assigned by the Director and Manager.

Requirements

  • Minimum age of 18 years old.
  • Pass and maintain a passable criminal background check and LEIE per state regulations.
  • Pass required controlled substance tests.
  • Ability to speak, read, and write effectively in English.
  • High school diploma or GED.
  • Complete all initial and ongoing training requirements within established timeframes.
  • Obtain and maintain SOAR certification (all employees).
  • Obtain and maintain Food Handler's Permit and T.B. test (Utah employees).

Essential Knowledge, Skills, and Abilities

  • Excellent public relations skills with individuals' families.
  • Effective communication with medical, professional, and state employees.
  • Good judgment and ability to handle crisis situations.
  • Strong interpersonal skills, including the ability to remain calm and maintain professionalism under pressure.
  • A service-oriented attitude.

Physical Demands and Work Environment

  • Physical demands: Walking, sitting, talking, hearing, using hands for various tasks, bending, stooping, lifting up to 35 pounds, restraining aggressive individuals, and lifting/moving an adult person.
  • Work environment: Exposure to various environments including aggressive individuals, exposure to blood and bodily fluids, and other Occupational Pathogens and Materials (OPIM). Must follow OSHA Standard Precautions.
