About the role
Assist with adults with I/DD disability in a day program setting. Implement individual Person Centered Plans involving skills that promote independence.
Responsibilities
- Instruct, model, assist consumers in daily living skills, socialization, leisure and/or recreational activities, educational and vocational skills.
- Read and be familiar with the Employee Handbook regarding personnel policies and procedures for the agency.
- Engage with and build a rapport with the person served.
- Determine consumer needs and implement the individual’s habilitation plan according to the service definition.
- Document the service provided in progress notes including observations, consumer participation, progress, behavior changes, or any pertinent information relating to the consumer.
- Ensure all documentation is completed on the day the service is provided and completed in BLACK INK only.
- Adhere to company policy prohibiting taking a person supported to a staff’s private residence or leaving the consumer unattended in a vehicle or at any other public place.
- Submit all required documentation by the deadlines specified by the Supervisor.
- Obtain and keep current all training required by the agency according to the service provided and individual’s needs.
- Attend all mandatory staff meetings and consumer conferences or provide advance notice to Supervisor if unable to attend.
- Receive supervision from Supervisor/Qualified Professional (QP) or Associate Professionals (AP) at least quarterly.
- Report changes in person’s condition, family situation, or needs to the Supervisor.
- Report any allegations of consumer abuse, neglect, or exploitation to the Supervisor for investigation.
- Report any destruction or theft of agency property to the Supervisor.
- Report any possible fraud against the consumer or agency to the Supervisor for investigation.
Requirements
- At least 25 years old
- High School Diploma or GED
- Able to read, write, understand, and follow directions
- No substantiated findings on the NC Health Care Abuse Registry
- Meet SBI and DMV requirements of LME/MCO contracts
- Valid driver’s license
- Three full years of driving experience
- Acceptable level of automobile liability insurance
- Experience with persons with intellectual/developmental disabilities preferred but not required
- Willingness to complete all required trainings as assigned
- Access to a device with internet connection at the time services are rendered
