Responsibilities
- Determines referral process, admissions, discharges, and transfers to other levels of care.
- Conducts in-depth clinical assessments and documents findings using approved forms.
- Participates in inter-agency meetings as needed.
- Performs pre-certifications and continues stay reviews with HMOs.
- Solves client funding issues.
- Completes assessments as needed.
- Orients family members to the nature of therapeutic services and the specific program.
- Collaborates in developing and implementing treatment and discharge plans.
- Coordinates staff schedules, transportation, and meals as applicable.
- Sends and tracks satisfaction questionnaires.
- Tracks operational data.
- Transports clients safely, as needed.
- Aids in planning and leading therapeutic groups and other activities.
- Communicates client progress and participates in treatment team planning.
- Completes progress notes, treatment plans, and discharge instructions/summaries.
- Documents client functioning, response to treatment, behavioral changes, and unsafe events.
- Collaborates with funding sources and treatment teams.
- Manages behavioral crises through observation, redirection, physical restraint, and team interventions.
Qualifications
- Minimum Education: Bachelors Degree
- Required Work Experience: Behavioral/mental health or social science experience, minimum 1 year as a Mental Health Worker or in a related field.
- Preferred Licenses: Basic Life Support upon hire.
- Required Driver's License: Statement required.
- Knowledge, Skills, and Abilities: Knowledge of current activities, therapy principals, and group counseling skills/techniques; analytical ability to manage tasks in a fast-paced environment; physical effort for patient assistance; visual ability for patient interaction and information management.
