Day Hab Team Lead-HCBS

About the role

Under the supervision of the Day Habilitation program Coordinator, the Day Habilitation Team Leader is responsible for ensuring delivery of quality services to all enrolled individuals. The Day Habilitation Team Leader is responsible for ensuring all DSPs complete all assigned daily tasks, routines and reports any concerns to the Program Manager. The Day Habilitation Team Leader will be part of an Interdisciplinary Treatment Team. This is a 40-hour per week position.

Responsibilities

• Under the supervision of the Day Habilitation Program Coordinator, the Day Habilitation Team Leader will foster independence in the individuals through Day Habilitation Action Plans.
• Complete daily communication log assignment sheets by assigning individuals and responsibilities to DSPs each morning.
• Ensure all assigned tasks such attendance, appointments and outings are completed.
• Ensures all billing and outcomes sheets are completed in a timely manner for each individual.
• Meets weekly and as needed with the Day Hab Program Coordinator to report any issues or concerns, review the past week and plan for the following week.
• Reviews all progress notes done in the system (ECR) on a daily basis as well as to ensure monthly summaries are completed and up to date.
