Day Center CNA

Responsibilities

• Works at Adult Day Health Center assisting participants with ADLs.
• Assists participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting.
• Documents care provided in accordance with established policies, guidelines, and applicable regulations.
• Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews.
• Assists participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed.
• Makes sure to monitor participants' health status such as vital signs, weights, skin condition, appetite and changes in functional ability and reports pertinent information to clinical supervisor in a timely manner.
• Collaborates with multidisciplinary teams in care planning for participants.
• Serves as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities.
• Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar when needed.
• Attends in-service and continuing education programs as scheduled and necessary.

Requirements

• Active and valid state-issued Certified Nursing Assistant (CNA) license.
• Current CPR certification or the ability to obtain CPR certification.
• High School Diploma or GED equivalent.

Preferred

• Experience in a care setting for frail and elderly patients participating in a group environment.
• Bi-lingual.
