Responsibilities
- Assists participants with ADLs (activities of daily living) and provides both restorative and supportive care.
- Follows the care plan and assists participants with daily activities such as bathing, dressing, grooming, hair care, mouth care, eating, and toileting.
- Documents care provided in accordance with established policies, guidelines, and applicable regulations.
- Carries out, reports, and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews.
- Ambulates, positions, transfers, cares for catheters, provides oxygen, and assists with other tasks as assigned or needed.
- Maintains awareness of participants' health status by monitoring vital signs, weights, skin condition, appetite, and changes in functional ability and reports pertinent information to the clinical supervisor promptly.
- Collaborates with multidisciplinary teams in care planning for participants.
- Serves as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments, supporting participants while in transit and accompanying them in and out of facilities.
- Creates and participates in group and one-on-one activities in accordance with the daily activity calendar when needed.
- Attends in-service and continuing education programs as scheduled and necessary.
- Performs other duties as assigned.
Requirements
- Active and valid state-issued Certified Nursing Assistant (CNA) license.
- Current CPR certification or the ability to obtain CPR certification from either American Heart Association or American Red Cross.
- High School Diploma or GED equivalent.
- Preferred experience in a care setting for frail and elderly patients participating in a group environment.
- Bi-lingual.
