About the role
Serving more than 10,000 Georgians, River Edge continues to be one of the state's leading resources for mental health, substance use disorder treatments and supports as well as individuals with intellectual and development disabilities. River Edge offers outpatient services at our Emery Highway-Macon, Monroe County and Baldwin County locations. Outpatient services include behavioral health, nursing, and psychiatric assessment, stabilization and medication maintenance services as well as crisis intervention services as well as awareness, education and recovery groups.
Responsibilities
- Engage clients and establish trusting, collaborative relationships directed toward the goal of competitive employment in community job settings with other workers who do not necessarily have disabilities.
- Provides a broad range of paraprofessional social services to patients/clients/consumers and their families, participates in treatment team planning, and performs crisis intervention.
- Conducts job development and networks with community business and leaders directed toward positions that are individualized to the interests of the team member’s caseload.
- Conducts a minimum of 6 employer contacts per week.
- Affords clients information about their benefits (e.g., SSI, Medicaid, etc.) and how they will be affected by employment for clients to make informed decisions about employment opportunities.
- Assists clients in obtaining information about their benefits (e.g., SSI, Medicaid, etc.) and how they will be affected by employment for clients to make informed decisions about employment opportunities.
- Assists individuals with reporting earnings, as needed.
- Provides individualized follow along support to assist clients in maintaining employment.
- Provides outreach services as necessary to clients when they appear to disengage from the service.
- Uses a variety of methods to provide outreach.
- Collaborates with Vocational Rehabilitation counselor to coordinate services for clients.
- May provide supportive counseling to consumers and families and/or serve as a liaison for social services.
- May perform case management duties, and skills training with consumers.
- Affords assistance in dealing with personal and social problems.
- Completes, maintains, and updates programmatic documentation/case records per DBHDD, IPS Fidelity, and VR guidelines.
- Must be willing to transport clients in personal vehicle.
Requirements
- Bachelor's degree from an accredited college or university in a social service related field (social work, sociology, psychology) preferred
- Two years of Social Service or related experience
- One year of experience working with Developmental Disabilities
- At least six months job coaching experience
