Cumberland Community Health Worker (Part-time/Temporary)
Serves as a bridge between healthcare providers and the community, promoting health education and access to services. Engages with individuals and families to assess health needs, provide information, and facilitate communication between patients and healthcare systems. Helps navigate resources and address barriers to care through outreach activities, health screenings, and advocacy for community health initiatives.
About the role
This is a part-time temporary, grant-funded position with a defined project end-date of October 30, 2026. The role is onsite, providing services at multiple community locations throughout Cumberland County. The primary work location varies based on program and operational needs, and regular travel between community sites is required. This position is not based at the organization's home office.
Responsibilities
- Conduct community outreach and engagement activities for rural health grant
- Work collaboratively with the project manager and team to maximize productivity
- Collaborate with the whole project team, contributing to the entire project lifecycle
- Document encounters, referrals, and follow-up actions in the case management database
- Maintain confidentiality and comply with HIPAA and agency policies
- Perform other duties as assigned to support project goals
- Utilize a tablet or other company-provided technology to facilitate this program
Requirements
- High School diploma required; working towards an associate or bachelor’s degree in a relevant field preferred
- Certified Community Health Worker in NJ (preferred)
- Valid driver's license and reliable transportation
- Ability to travel regularly between multiple community locations
- Ability to work a flexible schedule, including occasional evenings or weekends, if applicable

Pay
The hourly rate for this position is $25.00. This pay rate covers base pay only and excludes bonuses or other incentives.
