County Caseworker 2 (LG) Supports Coordinator - Wayne County MH/ID
About the role
Located in Honesdale, Pennsylvania, Wayne County seeks a compassionate, community-minded professional to join our team as a Supports Coordinator. This rewarding role involves supporting individuals with intellectual and developmental disabilities by helping them access services and stay connected to their communities. You will work closely with families, teams, and providers to ensure each person has what they need to meet their goals.
Responsibilities
Develop Individual Support Plans and review them regularly to match each person’s needs
Connect individuals with natural supports and necessary services based on their goals
Encourage participation in community activities and employment when appropriate
Travel to meet individuals and teams in person to review progress and needs
Complete documentation and data entry to maintain accurate and timely records
Work closely with families, supervisors, and peers to support each individual’s growth
Qualifications
Minimum Experience and Training Requirements: Six months of experience as a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or A bachelor's degree with a social welfare major; or A bachelor's degree which includes or is supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of professional social casework experience in a public or private social service agency; or Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences.
Other Requirements: PA residency requirement is currently waived for this title. You must be able to perform essential job functions.

Full-time employment. Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 1-hour lunch. Occasional overnight and out of county travel for meetings or training.
