Immediate Supervisor: Programs Coordinator

Responsibilities

• Intervene and evaluate consumers, establish preliminary diagnosis.
• Develop and monitor treatment plans collaboratively with consumers by defining goals, specific outcomes, and time-frames for treatment.
• Monitor consumer’s behavior and make detailed reports for medical necessity in regular comprehensive evaluations.
• Intervene therapeutically to change attitudes, beliefs, and behaviors of consumers to improve individual and family level of functioning.
• Provide crisis intervention to stabilize consumer’s condition by utilizing short-term crisis intervention techniques and/or referral to other support services.
• Maintain networking and referral contacts with other human service providers both internally and externally.
• Carry a caseload of clients enrolled in Community Support Team services and as assigned by direct supervisor.
• Complete required monthly direct service hours to consumers as defined on Direct Service Requirement sheet.
• Facilitate comprehensive consumer care by providing case management services and therapeutic intervention in community-based settings.
• Facilitate recreation/socialization and cultural activities for consumers which provide enjoyable outings that prepare and encourage them to engage in activities independent of the agency.
• Effectively teach life-management skills to consumers and assist with the development of independent living activities, such as grocery shopping, attending appointments, social skills training, money management, and coping skills.
• Provide and arrange transportation services to consumers which they translate to their natural environment.
• Demonstrate active involvement in supervision, team meetings, and clinical staffing.
• Collaborate with internal and external members of the interdisciplinary team to coordinate services and ensure the clients’ needs are being met.

• Behavioral Health Program(s): Community Support Team (CST)
• Behavioral Health: Outpatient Mental Health Programming helps individuals build life skills and well-being through behavioral health counseling, psychiatric care, and other care coordination and support services.
• SAMHSA CCBHC: Serves individuals living with a mental or substance use disorder who seek care, including those with severe mental illness (SMI), substance use disorder (SUD) including opioid use, children and adolescents with serious emotional disturbance (SED), individuals with co-occurring mental and substance disorders (COD), and individuals experiencing a mental health or substance-related crisis.
