Essential Duties & Responsibilities

  • Facilitates therapy sessions for a client or group of clients.
  • Aids client in developing an individualized service plan, ensuring it is person-centered, family-centered, and service-centered.
  • Maintains ongoing monitoring of client progress in treatment and participates in service plan reviews as assigned.
  • Updates service plans as necessary and submits them for medical review, if applicable.
  • Conducts intake for clients entering services, including explaining services, gathering necessary information, ensuring client understanding of forms, and submitting materials for processing.
  • Conducts assessments and writes summary reports, as assigned.
  • Aids clients in linking with internal and external resources as outlined in the service plan and in coordination with Integrated Healthcare Services.
  • Maintains communication with referral sources, providing updates on client progress, behavior, and needs (with appropriate releases), and engaging in ongoing consultation when necessary.
  • Aids clients in creating discharge plans and provides follow-up services as assigned.

Knowledge, Skills & Abilities

  • Effectively communicates via written, verbal, in-person, and virtual methods.
  • Skilled in customer service techniques.
  • Skilled in problem-solving.
  • Skilled in consultation and networking.

Qualifications

  • Education: Master’s Degree in a human services related field.
  • Experience: Experience providing mental health services preferred.
  • Certification/Licensure: Not applicable.

Pay Range

$48,300.00--$67,700.00

About the Role

Provides therapeutic interventions to promote personal growth and resiliency in clients with a primary behavioral health diagnosis. Performs continuing assessment of the client’s needs and functioning, maintains appropriate documentation, and develops and implements personal growth plans consistent with the client’s therapeutic needs.
