COTA - Travel Contract
Minimum Requirements
- Associate degree in Occupational Therapy from an accredited college.
- Active Illinois COTA license required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Prior experience working in a skilled nursing facility or long-term care setting preferred.
- Experience with restorative programs, ADL training, and use of common adaptive equipment preferred.
- Ability to collaborate with interdisciplinary teams and document patient care according to facility policies.
- Willingness to participate in a split assignment and float between nearby facilities in the local area as needed (split between facilities in Havana, IL and Cuba, IL).

- Shift: Days, 0830-1630.
- Schedule: 5x8.
- Assignment Length: 13 weeks.
