COTA / Occupational Therapy Assistant

Flagstar Rehab connects Occupational Therapy Assistants with assignments that align with their schedules and professional goals, offering LLC support for independent work. We provide daily updates on facility needs and handle administrative details so you can focus on patient care.

Responsibilities

• Collaborate daily with OT Supervisor and follow established Plan of Care for all patients.
• Provide quality therapy services to individuals in an SNF setting who require care related to TBI, CVA, Stroke, Parkinson’s, Multiple Sclerosis, Cancer, orthopedic surgery, and other debilitating diagnoses.
• Monitor and document patient progress.

Requirements

• Current NYS OTA License and Registration.
• Valid BLS Certification.
• EMR Competence (Proficiency in REHAB OPTIMA + POINT CLICK CARE a plus).
• Meets all State and Federal requirements for US employment in a Nursing Facility.
• Malpractice/Liability Insurance (if applicable).

Pay

Hourly Pay Rate:

• W2: $34
• 1099: $38-$40

Schedule

• Full-Time, Part-Time, Per Diem.
• Weekends, Days, Evenings, and Coverage Shifts Available.
