About the role

MJHS Home Care provides advanced clinical services and emotional support to patients recovering from illness, injury, or surgery at home. You will provide a full range of advanced clinical home care services, including patient assessment, medication management and patient/family education.

Responsibilities

• Assume ongoing, primary responsibility to coordinate, implement and continually evaluate the home care needs of your patients
• Review patient insurance information and coordinate activities involved in each individual care plan
• Ensure pertinent findings are shared with family and professional caregivers
• Create the initial comprehensive and interdisciplinary patient centered home health plan of care; update the home health plan of care in collaboration with the physician
• Effectively case manage the care and services for assigned patients resulting in positive clinical outcomes and minimized risks of rehospitalization
• Support agency established financial goals

Qualifications

• Current NYS RN license and registration
• Bachelor's degree in nursing preferred
• Minimum of one year acute medical-surgical nursing experience
• CHHA experience preferred
• Driver license and car required
