Coordinator HCBS - FT

About the role

RISE is an innovative human services network originally established in 1987 to move individuals living in institutions into family settings. Since then, services have grown to support children, adults, and families with a variety of needs across multiple states, including residential settings, day programs, employment assistance, managed care, and home and community-based services. The Service Coordinator oversees our Home and Community Based Services (HCBS) and Managed Care (MC) programs and is considered a safety-sensitive position. This role is ideal for a dedicated professional experienced in working with individuals with intellectual and/or developmental disabilities and the elderly, providing leadership and support to Direct Support Professional staff and the people and families we serve.

Responsibilities

• Manage and support HCBS programs by implementing the RISE mission, vision, and values, including hands-on experience to positively impact program support and operations.
• Supervise Direct Support Professionals (DSP) staff.
• Establish partnerships with RISE customers, including clients, families/guardians, and funding agencies.
• Oversee and manage details of company and program records.
• Complete and maintain payroll and billing documentation accurately and within designated time frames.
• Ensure programs meet licensing/survey requirements and contractual compliance.

Requirements

• High School Diploma or GED and at least a combination of two (2) years of supervisory experience.
• Must be bilingual, fluent in Spanish, with strong verbal and written communication skills.
• Experience working with the elderly and/or physically disabled in a long-term setting.
• Ability to work flexible hours.
• Self-directed, able to work with minimal supervision.
• Some travel is required within the county where HCBS services are provided or for in-state travel.

Additional Requirements

• Must possess a valid Arizona driver’s license, be at least twenty-one years of age, and meet driver requirements as specified by RISE Insurance Agency.
• Must successfully complete a background check, driving record check, drug test, and obtain a Level One Fingerprint Clearance Card.
