Contract LCSW/LMFT (Remote - Arizona)

Responsibilities

• Credentialing & Licensure: Participate in commercial and Medicaid payer enrollment processes.
• Complete and maintain CAQH and credentialing documentation.
• Review and sign provider enrollment applications and related attestations.
• Participate in occasional meetings related to network expansion and market launch.
• Participate in 1-2 user research interviews to understand the AZ landscape - provider and patient attitudes towards care.
• Identify local key opinion leaders in the pediatric space and facilitate introductions where appropriate.

Qualifications

• Active, unrestricted Arizona license as an LCSW, LMHC, LMFT, or Psychologist.
• Eligible for credentialing with commercial and government payers.
• Experience delivering behavioral health services in outpatient or telehealth settings preferred.
• Strong organizational skills and timely completion of credentialing requirements.
• Interest in supporting innovative, technology-enabled care delivery models.
• Average Arizona Medicaid enrollment preferred.
• AZ state residency required.
