Continuous Care Per Diem LPN (Camden / Burlington / Gloucester / Salem County, NJ)

About the Role
Ennoble Care is seeking a per diem Hospice Licensed Practical Nurse to work within and around Camden, Burlington, Gloucester, and Salem County, NJ, providing continuous care to patients. Your presence and services will bring comfort to individuals facing the end of their lives, helping them achieve acceptance and peace. A compassionate nature and excellent interpersonal skills are essential for this role. You should be knowledgeable in caring for the terminally ill, including administering medication and providing emotional support, with the goal of helping patients live their final days with dignity and comfort.
Responsibilities
- Per diem position with 12-hour shifts, supporting continuous care patients.
- Assist the Registered Nurse in carrying out the plan of care.
- Perform specialized procedures (as defined in the State Nurse Practice Act) under the direction of the Registered Nurse and physician’s orders.
- Assist with the preparation, implementation, and continuing evaluation of the patient plan of care.
- Actively collaborate with the Interdisciplinary Group (IDG) to ensure the patient’s plan of care remains current and addresses identified issues.
- Reinforce teaching to patients and family members regarding end-of-life care.
- Foster teamwork by understanding the roles of other care team members and participating in team and staff conferences.
Requirements
- Graduation from an accredited School of Practical Nursing.
- Licensed in NJ as a Licensed Practical/Vocational Nurse by the NJ State Board of Nursing.
- One or more years of nursing experience; hospice experience preferred.
- Must be a licensed driver with an insured automobile in good working order.
- Strong verbal and written communication skills.
- Proven experience as a hospice LPN.
- Commitment to safety precautions and high ethical standards.
- Sound judgment, attention to detail, and emotional stability.
- Compassionate, patient, and bilingual (a plus).
