About the role
The Consumer Services Coordinator (CSC) provides case coordination services for individuals with developmental disabilities and their families, adhering to the Lanterman Developmental Disabilities Services Act. This role operates within the West End Transition area, covering destinations such as Chino, Chino Hills, Upland, and more. Mileage reimbursement for business travel is provided monthly.
Responsibilities
- Coordinate services and supports for individuals with developmental disabilities and their families.
- Utilize Person-Centered Planning strategies to develop and execute Individual Program Plans (IPPs).
- Evaluate consumer progress and adjust IPPs as necessary.
- Advocate for consumers and their families, attending relevant meetings and preparing necessary paperwork.
- Seek out and utilize generic resources on behalf of consumers and their families.
- Complete individual or family crisis interventions and document activities.
- Complete Special Incident Reports and inform relevant parties.
- Coordinate consumer assessments and complete placement and liaison activities.
- Maintain Medicaid Waiver standards and ensure timely documentation.
- Arrange placements in residential facilities, day programs, and other training programs.
- Conduct facility audits and refer issues to Quality Assurance and Resource Development.
- Facilitate the purchase of services identified in IPPs.
- Prepare for compliance reviews and other administrative case reviews.
- Attend mandatory and other authorized trainings to earn CEU’s.
- Participate in case conferences and interagency meetings.
- Provide case coordination coverage and mentorship as needed.
- Visit Level 2, 3, and 4 residential facilities and day programs as assigned.
- Participate in Quality Assurance evaluations/audits of community care facilities, health care facilities, or day programs.
- Complete administrative requirements, Purchase of Service forms, and maintain consumer file information.
- Facilitate the purchase of services identified in IPPs.
- Ensure consumers' rights and dignity are maintained in service provision.
- Comply with IRC's personnel policies and procedures.
Requirements
- Bachelor’s degree from an accredited college or university with one year of experience in intellectual/developmental disabilities, social work, special education, early childhood development, or a related field.
- Master’s degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or a related field from an accredited college or university.
- Bilingual preferred but not required.
- Ability to establish and maintain effective working relationships with others.
- Ability to work cooperatively and effectively with others.
- Maintain good attendance and punctuality.
- Ability to follow oral and written direction.
- Good verbal and written communication skills.
- Full use of an automobile, valid California driver’s license, and liability insurance.
Qualifications
- Bachelor’s degree from an accredited college or university.
- Master’s degree in social work, psychology, public health nursing, teaching, special education, early childhood development, or a related field from an accredited college or university.

Pay
Hourly range: $26.3130 - $40.8201 per hour.
