About the role
Consonus Pharmacy is a closed-door long-term care pharmacy specializing in the needs of nursing facilities, assisted living residents, and their caregivers. We are currently hiring a Consultant Pharmacist to join our team that lives in the Eugene or Medford area!
Responsibilities
- Review resident medication regimens monthly (or as agreed upon) per patient acuity level and report findings in writing to facility administrator, director of nursing and physician
- Plan and present in-service education to other pharmacy staff and nursing facility employees as requested
- Understand and implement all appropriate pharmacy policies & procedures
- Participate in the development of emergency drug supply kits at each facility in conjunction with the medical director, provider pharmacy and nursing administration
- Liaison between facility and pharmacy staff
- Provide ongoing education directly or indirectly to residents preparing for discharge from facilities serviced
- Knowledge of CMS regulations and interpretive guidelines
- Attend quality assurance and other appropriate committee meetings as requested by facilities
- Prepare quarterly quality assurance reports
Requirements
- B.S. Pharm. Or Pharm D. degree from an accredited School of Pharmacy
- Current Pharmacy license in the state(s) of practice
- Ability to travel throughout a territory, including overnight stays as needed
Qualifications
- Experience in long-term care pharmacy preferred
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
